EMDR Part II Event Registration - Step 1 Event Date: 07/13/2012 8:15 AM - 5:00 PM 07/14/2012 8:15 AM - 5:00 PM 07/15/2012 8:15 AM - 4:00 PM Event Location: Virginia Beach, VA USA Agency Name: City of Virginia Beach Child & Youth Services Event Price: $375.00 Please note that as our agency is hosting the event, there will be a nominal fee of $10 per participant for drinks and snacks for the three days of the training. This fee is due the first day of the training, and can be paid by cash or check. This fee is NOT collected by EMDR HAP, and cannot be paid via their website. Your registration is conditional on payment of the training fee and receipt of both your signed Participant Agreement form (PA) and payment, postmarked no later than 2 weeks prior to the training (06/25/2012). Unless registration is "confirmed" by receipt of payment and completed PA Form two weeks prior to the Event's advertised start date, your seat may be assigned to someone else. Prior to your training It is suggested that you read the first eight chapters of Francine Shapiro's book: "Eye Movement Desensitization and Reprocessing: Basic Principles, Protocols, and Procedures, 2nd Edition". The book is available on the EMDR Institute site Registration Policy Cancellation Policy: All cancellations must be in writing, by email, fax or letter. Cancellation 14 Business Days (06/25/2012) Prior to Event Start Date: Refund of basic training fee, less a $45 administrative fee, upon receipt of a written cancellation postmarked no later than 14 days prior to the Event's advertised start date (via letter, email (training@emdrhap.org), or FAX 203-288-4060. Cancellation between 06/25/2012 and the 4th Business Day Prior to Event Start Date: 50% refund of basic training fee, upon receipt of a written cancellation postmarked no later than 4 days prior to the Event's advertised start date (via letter, email (training@emdrhap.org), or FAX 203-288-4060. Cancellation Within 72 Hours of Event Start Date: No refund for cancellations within 72 hours (3 days) of the Event's advertised start date. CEU's EMDR HAP does not apply for CEU's for the trainings, but the sponsor agency often does. Please contact the sponsor agency directly to inquire about CEU's. Some states and licensure boards allow individuals to apply on their own.
